One of the most common questions people ask when enrolling in Medicare is whether they have to pay a monthly premium for Medicare Part A.

The good news is that most people qualify for premium-free Part A. If you or your spouse worked and paid Medicare taxes for at least 10 years (40 quarters), you will generally receive Part A without paying a monthly premium.

Individuals who qualify for Medicare due to a disability, Amyotrophic Lateral Sclerosis (ALS), or End-Stage Renal Disease (ESRD) are also generally eligible for premium-free Part A.

However, if you do not have enough work history, you may have to pay a monthly Part A premium. In 2026, the premium is $311 or $565 per month, depending on how many quarters of Medicare taxes you or your spouse have paid.

Important: If you are required to enroll in Part A and do not sign up when you are first eligible, you may have to pay a late enrollment penalty. That’s why it’s important to review your Medicare eligibility and enrollment options before you turn 65 or become eligible for Medicare.
