An EOB is a document your health insurance company sends after receiving a claim for medical services you received. The claim may be related to a visit to a doctor, laboratory, hospital, or another health care facility.
Each time a health care provider submits a claim on your behalf, your insurance company creates an EOB. It lists the services and procedures provided during your visit, such as examinations, vaccinations, surgeries, or anesthesia services.
When you receive an EOB, review it carefully. First, confirm that your personal information, including your name and address, is correct. Then, make sure the services listed match the services you actually received. Errors in this information can affect how much you and your insurance company are responsible for paying.
Next, review how the costs were applied to your copayment, deductible, and coinsurance. These are the amounts you are responsible for paying under your health insurance plan.
If something appears incorrect, contact your insurance company and ask them to review the charges. If you still believe the amount is different from what your health care provider told you, contact the provider for clarification.
